I present myself as a Posture Pain Specialist therefore I usually start my observation section of SOAP with observable posture position then palpable hypertonic and hypotonic patterns.  If there is a known injury or loss I include how this pattern is supporting or avoiding condition. Assessment right?

Plan: Which modalities address the condition, sequence and position administered.  Back to assessment; what were the results of applying modality.  Back to plan; what future care is scheduled or requested.
